Virtual Spin Event Participation Agreement and Liability Waiver
Please read this agreement carefully before participating in the GH Foundation’s virtual spin event, Pedal Past the Stigma, Let’s Talk About Gut Health. By participating, you acknowledge that you have read, understood, and agree to the terms of this document.
Assumption of Risk
I, the undersigned participant, understand and agree that participation in any exercise program, including this virtual spin event, carries inherent risks of physical injury, including but not limited to muscle soreness, sprains, strains, falls, and, in rare cases, more serious injuries, such as heart attack or death. I am voluntarily participating in this event and knowingly assume all risks associated with my participation. I confirm that I am in good physical condition and have no medical conditions that would prevent me from participating safely.

I acknowledge that the organizers of this event are not medical professionals and are not providing medical advice, diagnosis, or treatment. This event and any instructions provided are for educational and entertainment purposes only and are not a substitute for professional medical care. I have been advised to consult with a physician or other qualified healthcare provider before beginning any new exercise program. I will not disregard professional medical advice because of anything I learn from this event.
Participant Responsibility
I am solely responsible for monitoring my own physical condition. I will stop exercising immediately if I experience any unusual or adverse symptoms, such as pain, dizziness, or shortness of breath. It is my responsibility to choose an appropriate level of activity and listen to my body throughout the event.
Safe Environment and Equipment
I acknowledge that I am responsible for my own safety during the virtual event and will ensure that my exercise space is suitable for physical activity. I will inspect the area for any hazards or obstructions before starting. I am also responsible for the condition and safe use of my own equipment, including my spin bike, and will follow the manufacturer’s guidelines.
Waiver of Liability
I hereby release, waive, discharge, and covenant not to sue GH Foundation, its officers, directors, employees, agents, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by negligence or otherwise, while participating in this event.
Technology Risks
I understand that I will not hold GH Foundation responsible for technical issues, such as audio or video disruptions, that may affect my participation in this virtual event.
Media Release
I grant the GH Foundation permission to use my image, likeness, or voice from event recordings for promotional or other purposes without compensation. If I do not wish to be recorded or have my image used, I will notify GH Foundation in writing prior to the event.
Jurisdiction and Governing Law
This agreement shall be governed by and construed in accordance with the laws of the State of Illinois, and any disputes arising under this agreement shall be resolved exclusively in the courts located in Cook County, Illinois.
Severability
If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
Electronic Consent
By clicking ‘I Agree’ or otherwise electronically consenting to this agreement, I acknowledge that my electronic signature is the legal equivalent of my manual signature.
Consent
By consenting or participating, I confirm that I have read and agree to all the terms of this agreement.
